Diabetes in the elderly and frail
Can digital help reduce low blood sugar level episodes (which result in hospital admissions) within care homes?
Working in partnership with Portsmouth Hospitals Trust, Diabetes consultants, nurses and care homes, this pathfinder is exploring ways that digital inclusion may be able to reduce hypoglycemic attacks in elderly frail patients in the area.
We are investigating ways that colleagues and patient digital inclusion can provide better outcomes for this cohort.
As a Wave 4 Pathfinder this will seek to scale the findings throughout the pathfinder journey.
